If you are a new client, please complete the Client Intake form and bring it to your first appointment.

The form is not savable unless you download it to your computer. However, you may still complete the form online and print it. If you prefer, a paper form will be provided.

Notice of Privacy Practices

Insight Clinical Services' privacy policy complies with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). HIPAA was updated in 2013 and now includes guidelines for how to use technology while respecting your privacy.

Please read the Notice of Privacy Practices, and ask you therapist if you have any questions or concerns.

Cancellation Policy

The cancellation policy was reviewed in your initial client intake packet. If you have additional questions or concerns, please talk with your therapist.

Current Clients

If you are a current client who needs to update your contact information, your insurance information, or your credit card information, please complete the appropriate page on the Updated Client Information form and bring it to your next appointment.

Consent to Release Information

Sometimes you may want your therapist to share information with someone on your healthcare team (for example, your physician, naturopath, or attorney), but by law your therapist cannot release this information without your written permission. Complete the Consent to Release Protected Health Information form to allow your therapist to share information.
